Chronic Disease Management
This chronic disease program supports strategies to improve multiple chronic diseases and the risk factors associated with certain public health priorities.

Community Health Assessment
The Shelby County Health Department conducts a community health assessment to determine strategic health priorities for Shelby County. The Department uses a process called M.A.P.P (Mobilizing Action for Planning and Partnership) to guide this process.
