About the Survey

- Demographic questions, including your zip code, age, and race/ethnicity, are essential for understanding how various issues may impact specific groups within Shelby County. By gathering this information, data can identify areas where interventions and resources are needed, and adopt a tailored approach to address localized community issues effectively. - Responses will be used to guide future community health improvement planning and funding distribution. Focus Groups:
In addition to CHNA survey data, SCHD is hosting focus groups to receive direct feedback from communities in Shelby County. A focus group is a group interview involving a small number of demographically similar people or participants who have common traits/experiences. Their answers and reactions to specific questions and conversations are noted and studied. Focus groups will be hosted until September 2023. Focus groups are a way to receive personal feedback from community members across Shelby County. The department hosts focus groups at local businesses and partners with community organizations to build feedback data over numerous zip codes.
