What is the Community Health Needs Assessment (CHNA)?
Health outcomes and quality of life are not solely contingent on genetics; how and where we live, work, play, learn, and worship also influence our quality of life and health outcomes. Therefore, understanding the social and environmental factors, in addition to the health status of Shelby County, is critical to the efforts of improving community health. A community health needs assessment, identifies key health needs and issues through systematic, comprehensive data collection and analysis.
The process involves community participation.
What is the Survey?
The CHNA survey creates an opportunity for community members to participate and help develop a plan to address health concerns impacting their community.
The SCHD is seeking feedback from the community to address the following:
- What are the health problems in the community?
- What factors create or determine health problems?
- What resources are currently available?
- What are the health needs of the community from a population-based perspective?
How can I participate?
The online survey is available in three languages to receive feedback from community members who speak English and from those who speak/understand Spanish or Arabic better. Those languages were chosen because they are the most commonly spoken languages in Shelby County.

Why should I participate?
Community participation is critical in identifying unique health needs and assisting the health department and partnering organizations with identifying health priorities the County should focus on over the next four years and determine how those resources should be allocated.
Participation in this survey is voluntary and will take less than 10 minutes to complete. Be assured that no identifying information about you will be collected. Please help us by sharing this survey with your patients, customers, and partners. All participants will have the opportunity to participate in the gift card drawing at the end of this survey.
