1. An original color coded high intensity soil map must be submitted with the application packet.

2. The fee is $175.00 for a subsurface sewage disposal permit.

3. A plot plan must be submitted that will identify any underground utilities, natural gas lines, and the location of the house, water wells or similar structures on it.

4. Also, the plot plan must show the exact location of the septic tank and field lines.

5. Along with the information listed above, a soil analysis for the lot must be sent with the plot plan in order to verify that the soil is suitable for a subsurface sewage system at the site.

6. In addition to the exact location being plotted on a plot plan the exact location of the field lines and septic tank must also be drawn onto one of the soil maps that are submitted for approval by the department.
